NOTES:
1. CEU Payment - After clicking on the ADD TO CART button, you will
need to return to this form and select the CEU payment button.
2. After PayPal payment has been processed, click on the 'Return to
Merchant'. That will take you to a page to register for the
conference. Please enter your name, address, and other registration
information
and submit.
Personal checks, money orders, and purchase orders accepted, as well as
credit card payments through PayPal.
Checks should be made payable to NETRA.
If paying by checks, money orders, or purchase order, please print the
registration page and mail with payment to

Purchase orders must have numbers issued prior to conference. (Requests
for purchase orders are not sufficient.)
A $15.00 fee will be charged for any check that is returned from the bank.
